Representative Accountability Database (RAD)

The Representative Accountability Database (RAD) is a registration system that validates the identities of service provider representatives performing transactions in the National Lifeline Accountability Database (NLAD) and the National Verifier.


The Representative ID is a unique number that connects the agent’s identity to the transactions completed in NLAD and the National Verifier. This number must not be shared with anyone besides the carrier(s) for which you work.


RAD ID

RAD registration is a two-step process

Representatives self-register for a Representative ID, which will be needed to perform transactions in NLAD and the National Verifier.


Representatives provide their Representative ID number to each service provider for which they work. Service providers will use the Representative ID to create the representative’s NLAD and/or National Verifier account credentials.

All new agents will be required to have a RAD ID. When creating the agent’s profile in the ST Admin tool, the Agency Administrator will enter it in the section “Representative ID” along with the rest of the information requested for the profile.

For all existing Promocodes, the list of RAD IDs assigned will be provided to the Elearning team and the information will be entered in the “Representative ID” section, using the EDIT option available for the promocode.

Registration Process


In order to obtain a RAD ID, the ST Agent will use the RAD ID website https://www.usac.org/lifeline/additional-requirements/rad/

The registration process must be completed by the ST Agent only. Personal information will be requested by the system, in order to complete the RAD ID Registration.

Once on the website, the agent will click on the “register here” button

The system will request an email address to be entered, in order to send the instructions to the agent

Once completed, click on “Submit”. An email with instructions will be sent to the email address provided

The email contains a link, where the agent will be requested to enter some information to continue:

The link is valid only for 72 hours. If that time passes and the agent has not used it, the registration process must be restarted.

The agent will be requested to enter personal information in the system, in order to continue with the process.

In order to complete the agent’s profile, security questions must be selected

After clicking the “submit” button, a new email will be sent to the ST Agent with the RAD ID information.
