HHW Process

In the State of California, the system will always request a HHW to be completed by the customer, as the state requires this step for all new enrollments.

After entering the customer’s information, the following pop up will be shown to the ST Agent

The customer will be required to answer a few quick questions in order to confirm that the household is not receiving another Lifeline benefit already.

The Quest App will let the agent know that the following section must be completed by the customer, explaining that the section is to confirm the household will be getting one benefit only. Once the customer taps on the I Accept option , the Next button will become available:

After all questions are completed by the customer, a final screen will show the HHW summary for the applicant to e sign:
