SSN Proofs
- SSN card
- W2 from within the last 2 years
- Prior year's state, federal, or Tribal tax return
- SSA-1099 form (Social Security Benefit Statement)
- Unexpired United States government, military, state or Tribal issued ID, which includes Social Security Number and/or Tribal ID
- Military discharge documentation
- Weapons permit
- Government assistance program document
- Statement of benefits from a qualifying program
- Unemployment/workers' compensation statement of benefits
The SSN card will always have to be signed, except if it belongs to a minor.
SSN only validates First name and Last name, Middle names/initials can be missing or not matching.
The SSN number from the proofs submitted must be identified as SSN. This means that if the title that comes before the SSN is Claim number, Member ID, etc., the proofs should not be considered valid.
Bear in mind that in Texas, the only valid SSN proof will be the SSN card.
