- Vault: applicants can call Customer Care and sign up over the phone. Before getting the phone, applicants will receive a pre-filled form to sign and send back. Applicants may also be requested to submit proofs of Program or Income qualification. The required documentation can be sent via mail or fax. Applicants can process an enrollment recording to avoid the need of going through the mailing process.
The Application Form recording and the Household Worksheet recording will be processed by a Customer Care agent through the phone. Please, check the Phone Numbers article for further information.
The Resend form escalation is available through Customer Care and through Safelink's website live chat. If needed, a Household Worksheet form will be sent along with the regular Application form.
- Web: applicants can sign up through our website by entering their personal information. This is known as e-sign, so applicants do not need to fill in a physical form, though they may be required to upload proofs.
- Street Team: it is a group of agents that creates applications on the street for Safelink potential customers. Street Team agents are led by 3rd party marketing agencies. These agents are provided with mobile devices to create Safelink applications and upload proofs, if applicable. They can also check the status of customers’ applications and some other related tasks.
How to guide customers to complete and return a Safelink Application Form:
- If it is a prefilled form, instruct the applicant to read the information on the first page and make sure that it is correct. If there is any mistake, they should not modify it by hand, but call us instead so we can correct it on our side.
- If it is an empty form, the applicant must fill in all the mandatory information.
- Indicate customers to read the legal disclosure carefully to date it and sign it afterwards. They may be required to put a checkmark next to each statement as well. Inform customers that by signing the form they are agreeing to the terms and conditions described in the document. If proof is required, inform the applicant to send copies (not originals) of the eligibility documents that proves the program participation or income level. If applicable, educate the enrollee about the different proof submission options.
Proof Submission Methods
- By Regular Mail: Applicants should place the form and, if applicable, a copy of their eligibility documents (not originals) into the envelope provided. The envelope has the correct address on it.
- By Fax: they can fax the form and eligibility documents at 1-866-902-5756. The Enrollment ID number must be included on the top of each page sent to us.
- Through the Web: applicants can upload proofs through safelinkwireless.com by logging in to their account (with the EID# and last 4 of the SSN) and select the Proof Submission section to find the Upload Proof option.

- By E-mail: The Enrollment ID number has to be included in the subject line, and the customer’s first and last name in the body of the e-mail. After attaching the proof, the e-mail has to be sent to myproof@safelinkmail.com

California LifeLine forms and letters are sent in PINK envelopes, exvept Braille-transcribed letters and forms.
Braille forms and letters are sent in white 9x12 and 10x13 envelopes becaues Braille-transcripbed documents are not folded.
