Unlike the NLAD States, Texas will have its own enrollment process. For this reason, the application form will also be different. This is how the header looks like:
As it can be seen, we will find the applicant's full name and address at the top left corner. Then, the information contained in the box provides clear procedures for applicants to know how to complete the form correctly and how to submit proofs properly, whenever needed.
In the case of the terms and conditions, they should also be initialized and not checked for them to be considered valid.
Applicants will also be able to provide a promo code in case they have one at the very bottom of the form.
On the second page, applicants will have to detail which plan they are interested in. They will be able to apply for the Safelink service using their own phone or <read plan from offer>. Only one option should be checked.
The second section of the page only has to be completed if the applicant is a minor or dependent. If that is the case, the full name, DOB and SSN information will be requested.
On the third page, applicants will have to provide their personal information (SSN, DOB, Full legal name, etc.). There will be a section to add a shipping address, a broadband provider for cases when the applicant applies through BYOP, and a checkbox to indicate if the applicant resides in Tribal lands:
After providing their personal information, applicants will be informed about the One per household rule, along with other instructions and information pertaining to the service. Finally, both qualification methods will be displayed. This means that there will not be one form for Income and another one for Program qualification. Applicants will only have to select one of them and provide the information requested.
if applicants complete both qualification sections, BO agents will have to select the qualification method based on the proofs submitted by the applicant. If the application form was submitted without proofs, it should be rejected, because it was filled in incorrectly.
In the last page, applicants will be able to introduce the information from the agent who helped them creating the application. Of course, this is optional and only applies if the applicant received help from a qualified agent.
Then, the Agreement section will present all the terms and conditions that the applicant will have to accept and initialize if they agree. Case contrary, the form will be considered invalid. Once again, applicants will have to provide their initials in each statement. No checks, symbols, etc. will be accepted.
Finally, the applicant will have to sign and date the application to finish with the form completion.
