Bear in mind that these are all the valid cards. If a card is not included in this list, it should be considered invalid.
From now on, in order to consider a Medicaid card valid, both sides of the card should be provided. If an applicant only submits the front of the card, the proof should be rejected. This applies for all channels, but does not apply for California applications.
If the front of the card is not showing the applicant‘s full name, the back of the card must contain the applicant’s signature. If not, the proof will be considered invalid.
The program cards presented should be readable and should not be modified, tampered or manually written.
If a card contains the applicant's full name and the initials AR, that document can be considered valid, since AR stands for Authorized Representative.
All cards that are not found in the catalog can be considered valid for DOB proof, if this type of proof needs to be submitted.
Digital/Scanned/Photocopied proofs are allowed for enrollments created through the WEB Channel.
If a Medicaid proof shows the applicants DOB, it should match with the one provided in the enrolment. Otherwise, the proof should be considered invalid and has to be rejected.
