CA - Proof of Life


Proof of recent life activity must display the following information:

  • The subscriber’s first and last name
  • Activity that has occurred within the most recent three (3) calendar months

Here are some examples of valid documentation that subscribers can submit as proof of recent

life activity:


  • A document issued by a government-administered assistance program such as CalFresh or Medi-Cal

  • A utility account bill (natural gas, water, sanitation, cable, electricity, or telephone)

  • Proof of income, such as; a paystub, a Social Security statement of benefits, a Veterans Administration statement of benefits, a retirement/pension statement of benefits, an Unemployment/Workers' Compensation statement of benefit

  • A mortgage or rent statement
