5) Device Options & SPIA Questionnaire
The following window will request the applicant to choose between the options available: in the example below, applicants can request the service by keeping their own phone or requesting a free SafeLink phone. Right after selecting either of the device options, the plan information will be displayed.

Note: If the 'Keep My Own Smartphone' option is selected, applicants will have to provide the device IMEI number.

If according to the IMEI provided the phone is not compatible, the following message will be displayed.

If the device is compatible, the following message will be displayed.

Below the Plan information section, applicants will find the Broadband Facts Labels, which provide the particulars on pricing, data allowances and limits, network speeds, and more.

For enrollments created in the state of California, applicants will have to read additional information related to the California Lifeline Program. If applicants agree with the terms provided, they will be able to continue with the enrollment process.

Demographic Information (SPIA Questionnaire)
In the state of California, it will be mandatory to send demographic information from customers in the SPIA methods, which stands for Security and Privacy Impact Assessment.
This new method implies adding a new page between the eligibility page and the review page, after choosing if the benefit applies to the customer, a customer’s child, or the customer’s dependent. This is how the page will look like.

As the image above shows, the following questionnaire will help to gather income and demographic information, which will not affect the user’s eligibility. Applicants can also select the “Prefer Not to Respond” option if they do not want to provide a specific answer.
For the first question (What is your Gender?), applicants will be able to select among the following options: Male, Female, Transgender, Non-Binary/Non-Conforming, Prefer Not to Respond.

In the second step, applicants will be asked about their Race and Ethnicity.

Depending on the previous selection, in some cases, the Ethnicity dropdown will become available to select one option from the ones provided.

In the third step, applicants will have to select one of the options provided in the list depending on the combined yearly income that applicants receive in their household.

As usual, if the applicant does not want to provide this information, they can select the ‘Prefer Not to Respond’ option located at the bottom. After answering all questions, the CONTINUE button will become available to continue with the enrollment process.
