INCOME PROOFS 
An income proof like the most recent Wage and Tax statement, a W-2 tax return form, a statement letter issued by the SSA or the most recent Form 1099. 
For income, you can send an SSA Letter showing your benefits, the last 3 months consecutive paystubs or the last year W2 form.
If your are not receiving any income, please request a “No Income Form” from the Social Security Administration (SSA).
Okay. These are the valid income proofs that you may submit: last three months consecutive paystubs, last year’s ITR, Last year’s W2 form or letter from the SSA stating your monthly benefits.
Since you applied through income, your State Agency will validate your application. The approval timeframe is XXXXX.

For Medicaid, you can submit a copy of your Medicaid card or a letter from the State showing that you are receiving that program.

These are the most common DOB proofs: Driver’s License, unexpired State ID Card,  SSA Letter, Birth Certificate and Passport.
SSN PROOFS
An SSN proof like the SSN card, a W2 form from the last 2 years, a Tribal tax return, an SSA-1099 form or any government assistance program document showing your SSN on it.
For SSN escalation, you can send a copy of the SSN card, SSA letter, W-2 form or any other official document showing your SSN.
Here is the complete list of valid SSN proofs: SSN Card, W2 Form, Priors State or Federal or Tribal Tax Return, SSA 1099 Form, unexpired US Government militay or State or Tribal issued ID, Military discharge documentation, Weapon's Permit, Government assistance program document, Statement of benefits from a qualifying program and Unemployment or workers' compensation statement of benefits and Eligibility confirmation from State eligibility database or administrator. Proofs should show the SSN and Complete Name. This should only be provided if the customer requests this information.

MMS SUBMISSION
For MMS submission, take a picture of your proof with your camera phone. On the body of the message, write the enrollment ID XXXX and send it to 60856. You can send one proof per message, which will be reviewed in up to 48 hours.
Let me inform you that SMS and MMS are two different things. You should submit your proofs through MMS.
MMS means Multimedia Message Service. They are frequently used to send photos and videos from camera phones.
MMS proofs will be received and processed in 48 hours.
MAIL SUBMISSION
