2) BO - Names Format
Basic rules
Proofs: the first and last name stated on the SSN, DOB, Address and Program proofs should match exactly with the ones provided in the application. This applies to all states except California.
First Name and Last Name: if the customer´s legal name has two last names, both should be included in the proof (except for FAP and CA enrollments).
Middle name: the middle name is optional and should not be taken into account. Enrollments with the middle name/initial introduced in the First name section will be considered invalid.
Pefixes & Suffixes: as long as they are separated from the name or last name, there is no need for them to figure both in the enrollment and in the proof. There is no need to include them. They will only be considered invalid if there is no spacing between them and the name (e.g. MichaelJr).
Compound first names: compound first names are accepted as long as the ones entered in the Application match with the ones on the proof. These can be included in the "First Name" Field, and they don't necessarily need to be separated by any special characters.
An example of this would be a customer whose first name is "Jose Luis". While it doesn't include any special characters nor is it separated by these, the naming convention is still valid. It's paramount that, as with all documents, the proof reflects the information in the Enrollment.
The following are some other examples that we consider valid:
- Any name beginning with the letters "DE" (e.g. De Oro Jackson, De Santos Martinez)
- Any name beginning with the letters "SAN" (e.g. San Juanita Smith, San Pedro Williams)
- Any name of Asian origin (e.g. Su Quin, Xiao Mei, Li Wei )
- Any name beginning with the letters "AL" (e.g. Al Bashir Hasan, Al Sharma Holmes)
- Any names with a dash or an apostrophe between them (e.g. Jodie-Ann Smith, Tammy-Lynn Johnson / D'Andre Guiness, La'Quanda Bishop, O'Malley Briant)
If a name is compound (e.g. Daniel-Lewis) both parts of the name should be present in the document to consider it valid. Many compound names can be identified by the hyphen that separates both names.
If you see any other case that you consider it should be added, let that know to your supervisor, so that they can forward the request and submit the information for review.
Special Characters
Accents: letter accents are not allowed (á, é, í, ó, ü, ñ). The system does not recognize these characters, reason why if the applicant's name has one of these symbols, they should be elided (e.g. Nuñez vs Nunez, López vs Lopez ,etc.)
If the first or last name in a proof is separated with a special character, like a hyphen (-), a dash (/), an apostrophe (‘) ,etc., in the enrollment it should be changed for a space.
Street Team queues
For proofs received through any of the ST queues (except for FAP proofs and CA enrollments), the golden rule is that the set of information provided must match exactly with the information introduced in the application.
This means that if the account has one name and one last name, the proof must show the same (except for FAP proofs and CA enrollments).
If the proof shows an additional last name, the proof will be invalid (except for FAP proofs and CA enrollments).
If the enrollment has an additional last name, the proof will be invalid (except for FAP proofs and CA enrollments).
If the first last name is placed in the First name section of a document, such as a Driver's license or a State ID, the proof is valid as long as the second last name is shown in the Last name section.
If last names are inverted, the proof is invalid (e.g. Smith Johnson vs. Johnson Smith).
Program - DOB - SSN - Address Proofs

Whenever we see that the applicant has two last names and the first one of them is in the First name section (as it can be seen below), the proof should be considered valid. This is for all states.
As long as the information introduced matches with the one in the enrollment, it is valid. The only thing to keep in mind is that the order of the names must be the same.
In the case above, if Joseph were the applicant’s first last name, the proof would be valid, as long as in the Last name section from the enrollment it was introduced as Joseph Sample. On the other hand, if the last name order in the enrollment were Sample Joseph, the proof would be invalid.
Forced Address Proofs


California Enrollments
For enrollments created in California, if the applicant has two last names or a compound last name, the proof/enrollment should be approved even if the enrollment/proof shows only one of the last names (the first or the second last name). This rule applies to all CA proofs.
