Pre-Recording explanation


I’ll be conducting a recording as a proof of your Household Worksheet.


During the recording, I'll be asking your personal information and read the applicable disclosure statements. Please be guided that the only acceptable responses to create a clear and clean recording are direct answers pertaining to the information being asked. If the question is answerable by YES or NO, kindly just respond “YES” if you agree. Otherwise, just respond “NO”. Only the first answer will be considered.


At any point that there is an interruption or background noise in the duration of the recording, I will have to stop and start all over again.

      • If the applicant answers with a NO, they can apply for Lifeline, because they live in a household that does not get Lifeline yet and the recording STOPS HERE.

      • If the applicant answers with a YES, read the second question.


2) Do they get Lifeline?

(Applicant will answer YES or NO)

  • If applicants answer with a NO, they can apply for Lifeline, because they live in a household that does not get Lifeline yet. The recording STOPS HERE.

  • If the applicant answers with a YES, read the third question.


3) Do you share money (income and expenses) with them?

(Applicant will answer YES or NO)



  • If applicants answer with a NO, they can apply for Lifeline, because they live in an address with more than one household and their household does not get Lifeline yet. 

  • If applicants answer with a YES, they do not qualify for Lifeline because someone in their household already gets the benefit.
